Permit applications have been filed with the city to build a 12-story apartment building in Jamaica on the site of a vacant church.

The building is slated to go up at 159-29 90 Ave., on the site of the former First Reformed Church, which now is holding services on Jamaica Avenue. The application was first reported by the real estate website New York YIMBY.

According to documents on the Department of Buildings website, the building will be 124 feet high and have 201 residential units.

The application includes 2,462 square feet dedicated to a community facility and 758 square feet for commercial use.

There is a request for 118 enclosed parking spaces.

The owner is listed on the documents as KD Sagamore of Dean Street in Brooklyn.
